This system level view is difficult to achieve because it is so dependent on a broad experience level - it depends on thinking beyond the subsystem level. But it is a vital capability for all key safety specialists regardless of their organization. It is critically important that we have a workforce with a broad and deep understanding of safety at the system level.

Lessons learned are a key part of improving our systems approach to safety. Even with a systematic approach to lessons learned, you will not be able to accurately analyze previous experience and draw conclusions for future direction unless you understand that lessons learned are products of system interactions.
