Root Cause Analysis - Continued
Root causes can be defined as the first factor in a chain of events that can be controlled through a regulation, policy, procedure, or standard. It is a point in the chain of events at which internal control can be exercised. A cause is a set of conditions; each necessary but only together is it sufficient.
Simply put, causes can be found by stating the end result. However, you must keep asking "why?" until you have found a factor that can be corrected by the application of a regulation, policy, procedure, or standard at the governing, management, implementing, or individual level, or you have reached a non-correctable situation. There may also be insufficient data to proceed further.
